AGREE TO PARTICIPATE IN A RESEARCH STUDY
Title of Research Study: Design of an Alternative 911 Mobile Communication System Concept

You are being asked to participate in a research study because you are the target audience for who would most likely use the product, and you may have experience with the existing 9-1-1 system.
Purpose:
The purpose of this research study is to investigate and research the existing 911 communication system and design a mobile app integrating the standard system with new services, features, and technologies, such as gps location capabilities, video conferencing, language/ translation, usability/accessibility considerations, etc. The mobile app 911 interface design will be optimized to support quick response requirements, and use by those with disabilities. The research outcomes and deliverables would include a prototype interface design, an updated system and operational workflow.
Procedures:
If you volunteer to participate in this research study, we will ask you to do the following:
- Complete the following questionnaire, online, which should take approximately 15 minutes.
- The questions include personal demographic information (age, race, language) and any past experiences with the current 9-1-1 Emergency Communication System
- We are looking for recommendations for improvement of the system utilizing existing and future technologies not currently in use.
Time Commitment:
Your participation in this research study is expected to last for a total of 15 minutes.
Potential Risks or Discomforts:
- The subject may experience minor discomfort or distress when recalling their experiences and prior contact with the 9-1-1 emergency communication system.
Potential Benefits:
- The benefit in participation are the potential outcomes of designing an efficient state-of-the-art 9-1-1 Communication System that could improve response times, and/or directly solve problems without delay.
You will not receive any payment for participating in this research study.
New Information:
You will be notified about any new information regarding this study that may affect your willingness to participate in a timely manner.
Confidentiality:
We will make our best efforts to maintain confidentiality of any information that is collected during this research study, and that can identify you. We will disclose this information only with your permission or as required by law.
We will protect your confidentiality by storing all data on a secure, password protected server accessible only by the researchers. No personal, identifiable information will be collected.
The research team, authorized CUNY staff, CUNY Research Scholars Program (CRSP), and government agencies that oversee this type of research may have access to research data and records in order to monitor the research. Research records provided to authorized, non-CUNY individuals will not contain identifiable information about you. Publications and/or presentations that result from this study will not identify you by name.
Participants’ Rights:
- Your participation in this research study is entirely voluntary. If you decide not to participate, there will be no penalty to you, and you will not lose any benefits to which you are otherwise entitled.
- You can decide to withdraw your consent and stop participating in the research at any time, without any penalty.
